My guess would be that is one cylinder in a bank for maritime application.  But just a WAG.

Nope, not seen this. Eight stroke engine then? Eight strokes/crank revolution. I'd like to know what the advantages and disadvantages are.

  Something rings a bell about a very expensive Ducati single 20 or so years back but I could be way off

Atkinson engine design.  The Toyota Prius (for one) uses an engine of this design.

The best part is you can easily make one yourself if you feel the need. All you need is a twin engine and a bolt on section for the balance beam and the balance beam itself. Pretty easy and sorts the major primary imbalance.

Atkinson Cycle.  Variable stroke, the power and exhaust strokes are longer than intake and compression due the angle on the jack shaft (or whatchacallit). The cam is run on the mainshaft.

Can't say about the Prius; but the "Atkinson " V6 in my Tacoma is an emulation of the mechanical one (as above) by using variable cam timing. Seems to work as I get 22+ mpg overall and has since new.

If I look at the picture is it so that this system was used on the albatross racer from Guzzi also?

No the Albatross had no imbalance compensation. No Guzzi single did.
