I'm not to sure about "Hacking" a small block.

The V9 is rated at 55 horsepower. My wife and I rode a couple of BMW twins with EML sidecars for many years and they had similar horsepower. They moved along with the freeway traffic in Southern CA with no problems.
IMO; shouldn't be a problem.

Cool, I want one. But with some color somewhere. And I'm sure the engine would handle it, but would the frame? I'd contacted Piaggio about the V7II having a tub atrached and they said don't do it, the frame isn't designed for it. Probably in how the frame comes apart, there is no good way to reinforce those points to handle the stresses of the sidecar. Not to say people haven't done it...

Yeah, I know of one in SoCal. Just the same, I wouldn't do it.

Yep, the frame couldn't handle the stress of a sidecar without adding a sub-frame. DMC (Dauntless) Sidecars would probably make one for you.

Kind of sparse on fenders🏍

I like everything about it except for the typically excepted components left out to make projects like these look good/better:
Fenders
Mirrors
Indicators
Flush those in with the design and it's a home run to me.

I'm not too impressed with the position of the ignition switch!
Might need riden home in the side car after emergency braking.
(If only they had had Raleigh Choppers in Italy, with central gear shifter: the designer may then have known better)

This bike is one of many modified Guzzis from the Italian MC show called Lord of the Bikes. All of them customized off the V9 platform.
I found this link with a lot of trailers to different shows with numerous customized V9s. Two groups go up against each other to build a customized then they pick a winner.

I screen shot a few of them for your enjoyment or horror depending on your perspective.
